Graduation
Each prospective graduate must file for graduation. The Graduation Application must be completed and submitted for processing.听 Please see Graduation Application Instructions for more details. Please direct all questions regarding graduation procedures to the Graduation Unit of the Office of Admissions & Records or the SLP Program Director.
Credential Request
Preliminary Administrative Services Credential
Once you have completed all of the credential coursework requirements and can verify passage of appropriate examinations use this form to request recommendation from CSUDH for your online CA credential process.
Verification of experience MUST be on the district or employing agency letterhead and signed by the superintendent, assistant superintendent, director of personnel, or director of human resources or designee. School or district personnel other than the application must verify all experience.听To be submitted with your Credential Request Form. Used to verify minimum of five years teaching experience.
Preliminary Administrative Services Credential - Verification of Employment as an Administrator (Form CL-777)
Required for preliminary applicants only. To be submitted with your Credential Request Form. Used to verify employment as an administrator.
Clear Administrative Services Credential (Formerly Tier II)
Once you have completed all of the credential coursework requirements and can verify passage of appropriate examinations use this form to request recommendation from CSUDH for your online CA credential process.
