Submitting Documents
You may submit verification documents (tasks), including some appeals, by logging into your MyCSUDH at and accessing Financial Aid Student Forms.
Financial Aid Student Forms allows you to complete your documents with an e-signature and is mobile friendly, so you can complete tasks using your phone and even opt in for text reminders.
How to access the Financial Aid Student Forms
You must create an account to access Financial Aid Student Forms for the first time. You'll be asked for some basic information when creating your account which is used to validate your identity so that electronic signature can be used on future forms.
Create your account in four easy steps:

**For CADAA applicants, please use your CSAC Application ID to create your account (found in your CADAA Application). Do not use your assigned SSN or ITIN.
Please Note: Determination of aid eligibility cannot occur until all items on your To Do List are received by our office and removed from your To Do List. We will process your financial aid within five to seven business days of receipt of all forms and supporting documents.听
Before you submit your documents, combine all documents as one PDF, add your signature, and upload the documents.
