The following forms are to be completed by a licensed professional.

A licensed healthcare professional must complete the form if you have an established treatment relationship lasting at least one month.
鈥淯nacceptable forms of documentation include, but are not limited to, notes written on prescription pads, vague statements that a licensed medical or mental health professional is "prescribing" an emotional support animal, letters written by online agencies, or letters by professionals having no therapeutic relationship with the resident.鈥�
