Registering in the Defensive Driver Program

Complete the Defensive Driver Program Registration Form and attach the INF 1101 鈥� Authorization for Release of Driver Record Information (EPN Form). For employees with an Out-of-State Driver's License, complete the Defensive Driver Program Registration Form and attach the INF 1102 form.听

For existing employees whose job duties change or who now need to drive for 鈥渙ne-off鈥� reasons, complete a copy of both forms and submit to Risk Management via .听

The INF 1101 (EPN) form or the INF 1102 form (Out-of-State Driver's License) will be provided by Human Resources to eligible employees as part of the onboarding process.

For the INF 1101 Form:

• Employee completes Section 1 (Driver Information) ONLY. EPN Administrator completes Section 2 (Authorized Representative Certification).
• Per CA DMV, the form will take a minimum of (7) working days to process.

For the INF 1102 Form:

• Employee completes Section 2 (Driver Information) ONLY.听
• Per CA DMV, the form will take a minimum of (10) working days to process.听
• The INF 1102 form does not require a processing fee.
